Travel Team Registration


You may Register by Mail - All Registration forms will be available on the AHA Forms Page.
Just download the Travel Player Registration Form, Waiver of Liability Form, and the Consent to Treat/Medical Form  and mail them with your payment to Rob Alber, P.O. Box 342, Deerfield, MA 01342


White/Blue Jerseys: Travel Team jerseys may be ordered at registration or later through Division Directors once the team rosters are established. For new or replacement jerseys, you may contact your team representative or call the Association's equipment manager, Eliot Deres at 549-6724 ( or email: eliotderes@amhersthockey.org ).

You must have aWhite and Blue jersey for Travel . The cost of each white or blue jersey is $42.00 (price subject to change). For replacement jerseys, please provide previous jersey size and number. Players will be required to own red, white and blue striped socks for all AHA Games. Match the predominant color with your jersey color.


Please note: The Amherst Hockey Association is a volunteer organization that is partially funded by membership fundraising activities. All players are required to participate in fundraising activities to benefit the AHA. If you have any questions about fundraising obligations, please ask an AHA volunteer.
